Should we run a new trial of carotid surgery/stenting vs optimal medical therapy, and if so, how?

We are planning a research trial to determine the effectiveness of carotid surgery/stenting vs modern optimal medical therapy. The aims of the survey are to determine if such a trial is required and help us design a trial to ensure maximal patient benefit and clinician engagement. It should take about ten minutes to complete, there is only one mandatory question so if you just want to answer that it will take about 30 seconds.
